Why Pain Can Feel Sudden, Even When It Has Been Building for Weeks

Over the past few weeks in practice, across both our Doncaster and Toorak clinics, I’ve noticed a pattern that has been repeating itself for years.

People often come in and describe something that feels like it has just happened. A back that suddenly became painful, a neck that felt restricted overnight, or a shoulder that did not respond well to a simple movement. There is usually a moment they can point to: a weekend activity, a stretch, a turn, or something small that seems to explain it.

 

But when we slow things down and take a proper look, it is rarely just that moment.

What often emerges instead is a sequence. A few weeks of sitting a little more than usual. A change in routine. Less movement, or a different kind of movement. Small adjustments the body has made without much complaint. Nothing dramatic, and nothing that would necessarily prompt someone to seek help at the time. It builds quietly in the background until one day, something becomes noticeable.

After many years in practice, working with people throughout Manningham and in Toorak Village, that is where my attention usually goes first. Not just to what happened, but to what may have been leading up to it.

The body is remarkably good at adapting, but those adaptations can have consequences. One area may work a little harder, another may move a little less, and over time a pattern can form. Pain is often the last part of that story, not the beginning.

I have seen the same principle play out over decades of karate training.

In the dojo, you might watch someone struggle with a movement in a kata or lose their balance in a stance. At first glance, it looks like a mistake in that moment, something that just needs to be corrected. But with a bit of experience, you start to see that the issue may not have begun there.

It might be a lack of stability through the hips, or tension that has been building over time. It may even be fatigue changing the way the body holds itself under pressure.

A good instructor does not just fix the visible error. They step back and look at what is underneath it. They adjust the stance, refine the posture, and sometimes take the movement apart completely before building it back up again.

When that underlying pattern changes, the movement can often become more natural, more efficient, and less effortful.

That approach has shaped the way I work in practice.

Sometimes the answer is straightforward. At other times, it takes a bit longer. We move through things together, looking at what feels restricted, what may not be moving well, and what may have been compensating along the way.

Chiropractic care may involve adjustments, movement advice, or other approaches depending on the person’s presentation, comfort level, and clinical needs. Often I will get down on the floor with a patient and show them, because feeling a movement is very different from having it explained.

There is no real shortcut to that process, and there is no need to rush it. When we understand the broader pattern, care can usually be planned in a more specific and measured way, with advice tailored to the person rather than just the painful area.

In long-term practice, you often get to see how people’s movement patterns, work demands, habits, sport, and daily routines change over time. That history can be useful, because pain rarely exists in isolation. It often sits within a broader pattern, and understanding that pattern can help guide more appropriate care.

This is the work as I see it. Not just paying attention to where it hurts, but trying to understand the factors that may be contributing to it.

We will keep paying attention to those patterns, and we will keep working through them carefully when they show up.
